I, the Applicant hereby expressly give Dis-Chem Pharmacies Ltd (Dis-Chem) permission to collect and process any of my personal information as defined in the Protection of Personal Information Act (No. 4 of 2013) ("POPIA") and the associated regulations and this can be done -
1.1 for any legitimate purposes connected with my potential employment, including but not limited to maintaining personal contact details, to comply with applicable legislation, occupational health and safety, security and access control, employment and other credit references, and the like;
1.2 in order to comply with laws and other measures designed to protect or advance persons, or categories of persons, disadvantaged by unfair discrimination;
1.3 in order to protect Dis-Chem's legitimate interests;
1.4 in order to perform obligations under law or contract, to protect my legitimate interests or to pursue the legitimate interests of Dis-Chem or of a third party to whom the information is supplied.
- For purposes of this clause, "processing" refers to processing as defined in POPIA and includes but is not limited to collecting, receiving, recording, organising, collating, storing, updating, retrieving, altering, using, disseminating, distributing, merging, linking, blocking, degrading, erasing or destroying of any personal information.

- I warrant that any and all personal information provided by me to Dis-Chem shall at all times be true and correct and that the provision of inaccurate and/or misleading personal information shall constitute a serious offence.
I confirm that Dis-Chem may retain my personal information in their database for future matching and employment opportunities.
If I do not wish Dis-Chem to retain my details at any stage, I will email Dis-Chem directly ([email protected]) so that they can delete my details from their database.
